List of Documents

The list of documents required for the provision of public services when a service recipient applies to a service provider: (the legislation does not provide for, the list of documents is based on paragraph 6 of Article 26 of the Air Defense System “On Education”).

1) an application for admission in any form;

2) the original of the educational document;

3) 3×4 cm photos in the amount of 4 pieces;

4) medical certificate form 086-U with the attachment of a fluorography

(for the disabled of groups I and II and disabled since childhood, the conclusion of a medical and social examination), approved by Order of the Acting Minister of Health of the Republic of Kazakhstan dated November 23, 2010 No. 907 (registered in the Register of State Registration of Regulatory Legal Acts for No. 6697);

5) Certificate of unified national testing or comprehensive testing (if available);

6) an identity document (for identification purposes).

The documents certifying the identity of the service recipient must be presented personally or by legal representatives.
